NHS Grampian has apologised after being fined £70,000 over the death of 89-year-old patient Patrick Brogan, who was repeatedly given food that did not meet his special dietary requirements at Aberdeen Royal Infirmary.
Between 29 December 2023 and 6 January 2024, the health board failed to ensure a safe system for preparing and serving appropriate meals to patients with swallowing ifficulties. On 1 January, Mr Brogan choked after receiving unsuitable food. He was later given another inappropriate meal on 6 January, developed aspiration pneumonia after food entered his lungs, and died six days later.
NHS Grampian admitted health and safety failings in court and was fined £70,000, together with a victim surcharge of £5,250.
Following the case, chief executive Laura Skaife-Knight issued an unreserved apology to Mr Brogan’s family and the local community. She said the health board had carried out comprehensive review and introduced several measures to prevent similar incidents. These include a revised mealtime coordinator role, a colour-coded tray system, stronger nursing handovers, and improved staff training to ensure patients receive meals that meet their dietary needs.
